Hours Required for Certification: 570 from Board approved massage therapy school and a written state exam

Type of License/Title: LMT (Licensed Massage Therapist)

Certification Test: Written State Exam

Fees Required: Application $50 / Exam $80

Renewal Process: $120 for two years for license / $20 every two years for apprentice permit
